A note about complex bulbous rhinoplasty cases

These techniques are especially important in more complex cases, such as revision rhinoplasty or patients with thicker skin. In revision cases, the nasal tip may have been previously over-reduced or lack adequate support, requiring careful reconstruction using cartilage grafting.

Similarly, in ethnic rhinoplasty, the goal is not to reduce or erase defining characteristics, but to enhance tip definition while preserving the patient’s natural identity. In both scenarios, a structural approach allows for refined, balanced, and long-lasting results.

Why an Open Rhinoplasty Approach Is Often Best

An open rhinoplasty approach is often best for refining a bulbous nose tip because it allows direct visualization of the underlying cartilage, enabling greater precision, improved symmetry, and more predictable, long-lasting results.

This technique involves a small incision at the base of the nose, allowing the skin to be gently lifted so I can directly visualize the underlying cartilage. This level of access is critical when performing detailed tip refinement because it allows for:

  • Better visualization of the lower lateral cartilages
  • Precise placement of sutures and cartilage grafts
  • More control when reshaping and supporting the nasal tip
  • More predictable healing and long-term structural stability

While closed rhinoplasty has its place, it does not provide the same level of visibility or control for complex tip work. In my experience, an open rather than closed rhinoplasty approach is often preferred when the goal is meaningful, natural-looking refinement of a bulbous tip.

What Should You Expect During Recovery?

After bulbous nose rhinoplasty, most patients experience swelling and mild bruising, particularly in the first 1 to 2 weeks. While you may feel comfortable being seen in public within a few weeks, tip definition continues to improve gradually over several months as swelling resolves.
